Know Your Insurance Benefits
It is important that you understand your benefits. Check with customer service, human resources, or your agent for clarification of your benefits and responsibilities. We can provide billing assistance but it is your responsibility to contact your insurance company for a complete explanation of benefits.
Some insurance policies require that a deductible be 'met' before coverage begins. You are required to pay that amount before expenses are covered.
Your insurance company may require you to pay a specific percentage of the day's visit. This will be collected at the time of the visit or you may receive a bill later.
A set amount for each visit is determined by your insurance provider. You are required to pay this at the time of your visit. You may receive a letter for balance due. If you are unable to pay it entirely, please ask for a payment plan.
You will be expected to pay in full if do not have insurance, or we do not participate with your health plan or if we are unable to verify your insurance coverage at the time of service.
Charges for self-pay patients
